
Specialist Support Coordinator – Task Description
Care 4 Disability
As a Specialist Support Coordinator at Care 4 Disability, you will deliver high-level, intensive support to NDIS participants with complex needs, ensuring they can effectively access and utilize their NDIS plans to achieve their goals. This role focuses on reducing barriers, coordinating multidisciplinary supports, and empowering participants to navigate complex service environments.
Key Responsibilities:
Comprehensive Needs Assessment
Conduct in-depth assessments to understand participants’ goals, challenges, and support needs.
Collaborate with participants, their families, and carers to identify barriers impacting access to supports.
Service Plan Development
Design personalized service plans to address complex needs, ensuring alignment with NDIS “reasonable and necessary” criteria.
Prioritize immediate support needs and develop strategies to overcome significant barriers.
Coordination of Supports
Connect participants with NDIS-funded supports (e.g., occupational therapists, psychologists, support workers) and mainstream services (e.g., health, housing, justice).
Coordinate multi-disciplinary teams to ensure seamless service delivery, avoiding gaps or overlaps.
Facilitate transitions, such as finding new providers or upskilling support workers, to meet participants’ specific needs.
Empowerment and Capacity Building
Support participants to build skills for self-advocacy, decision-making, and independence.
Provide guidance on navigating the NDIS system and accessing community or government services.
Monitoring and Reporting
Regularly monitor the effectiveness of supports and adjust plans as needed.
Prepare detailed reports for the NDIA, including recommendations for plan reviews and future support needs.
Ensure compliance with NDIS Code of Conduct and pricing arrangements.
Crisis and Risk Management
Proactively identify and address risks to participants’ safety or wellbeing.
Develop strategies to manage crises or unexpected situations, ensuring consistent service delivery.
Stakeholder Collaboration
Work closely with participants’ support networks, including families, carers, and other providers.
Liaise with the NDIA, Local Area Coordinators (LACs), and other stakeholders to advocate for participants’ needs.
Qualifications and Skills:
Essential: Relevant tertiary qualifications (e.g., social work, psychology, occupational therapy, or related allied health fields).
Experience: Proven experience working with complex populations, preferably in disability, mental health, or justice systems.
Skills: Strong problem-solving, communication, and interpersonal skills; ability to navigate complex systems; empathy and cultural competence.
Knowledge: In-depth understanding of the NDIS, its processes, and the NDIS Code of Conduct.
